This graph shows the stages of establishing a system of care in theory. The x axis represents time in years, and the y axis represents level of effort. Two lines are drawn in the graph: One for building the system and one for sustaining the system. The graph indicates that building the system requires more effort during the first 3 to 4 years, with effort peaking around year 2. Level of effort for building the system decreases from that point forward. In contrast, the graph indicates that sustaining the system requires less effort during the first 2 to 3 years, with effort increasing from that point forward. The two stages overlap briefly around years 3 and 4, but otherwise are depicted as separate efforts.

The graph indicates that building the system requires a high level of effort during the first 3 to 4 years and may increase in subsequent years due to factors such as a change in agency leadership. The graph also shows that plans for sustaining the system must start within the first 2 years of establishing the system and sustainability efforts must continue throughout the life of the system. Overall, the graph depicts that efforts for the two stages should occur simultaneously, and the level of effort fluctuates for each stage.
