On behalf of the Children’s Bureau, the Child Welfare Information Gateway and the Capacity Building Center for States hosted a series of webinars to perpetuate the learning from the Youth At-Risk of Homelessness (YARH2) grantees. The Children’s Bureau funded the YARH multi-phase grant program to build the evidence base on what works to prevent homelessness among youth and young adults who have been involved in the child welfare system.
The fourth webinar in the series highlighted the lessons learned and shared resources and tools from Alameda County Social Services Agency’s project, Alameda County Youth Transitions Partnership (YTP). The Alameda County Social Services Agency, in collaboration with community partners, developed YTP, a model that blends service coordination, intensive case management, and dialectical behavior therapy to help youth in foster care engage with available support systems and improve their outcomes.
