Systems of Care Toolkit

This toolkit provides examples of critical components of systems of care that have been developed by communities across the country. Many of these resources were initially created during the early development of systems of care. Although they may focus primarily on the mental health needs of children, they are adaptable and appropriate for use within all systems of care for families seeking support.

Policies that promote systems of care
Federal laws, State statutes, interagency agreements, and other policies that help community organizations address the multiple needs of children, youth, and families.

Systems of care infrastructure
Examples of management information systems, communications and funding strategies, assessment tools, curricula and training materials, and governance structures that help ensure all children, youth, and families are served within their home communities in a culturally competent, strengths-based, and comprehensive manner.

Systems of care services and supports
Program descriptions for systems of care services such as respite care, crisis outreach, individualized or wraparound care, therapeutic foster care, and others.

Evaluation of systems of care
Evaluations of the systems of care approach and information on how to evaluate program effectiveness.

A capacity enhancement curriculum to strengthen the knowledge base and skills of leaders involved in building systems of care. Addresses critical thinking, strategic frameworks for system building, and the elements of an effective system building process.

University of Kansas, Work Group on Health Promotion and Community Development
Provides thousands of pages of practical skill-building information on more than 250 different topics, including step-by-step instruction, examples, checklists, and related resources.

North Carolina System of Care Toolbox
North Carolina Child and Family Mental Health Services
Helps communities design and sustain a system of care approach for providing services to children and families. Resources address forming collaboratives, partnering with families and youth, cultural competency, and building capacity through training and technical assistance.
