Appendix D: Sample Logic Model Description

Appendix D shows a sample logic model.

The logic model includes three rows of boxes. The first row shows four boxes which flow from left to right (represented by arrows connecting each box). The first box includes the word "Inputs"; the second box contains the word "Activities"; the third box contains the word "Short-term Outcomes"; the fourth box contains the word "Long-term Outcomes."

The middle row includes one long box containing four words: "Resources," "Services," "Immediate Outcomes," and "Longer-Term Benefits."

The bottom row shows four boxes which flow from left to right (represented by arrows connecting each box) and are examples of resources, services, immediate outcomes, and longer-term benefits.

The first box contains the following resources:

  • Office on Child Abuse and Neglect (HHS)
  • Annie E. Casey
  • DHR
  • Title IV-E

The second box contains the following services:

  • Multi-Faceted Home Based Intervention
  • Emergency Services
  • Family Assessment
  • Outcome-Driven, Tailored Services

The third box contains the following immediate outcomes:

  • Increase Protective Factors
  • Decrease Risk Factors

The fourth box box contains the following longer-term benefits:

  • Child Safety
  • Child Well-Being
