Medicine and the Prevention of Child Abuse and Neglect (Workshop 99)
The general pediatrician is in a prime position to play an expanded role in the primary and secondary prevention of child abuse and neglect. Anticipatory guidance is a regular part of pediatric care providing the context for teaching healthy child development, augmenting parenting skills, and identifying and supporting vulnerable families.
This workshop presents the methods and findings of Practicing Safety, an innovative research project designed to decrease child abuse and neglect by enhancing anticipatory guidance provided to parents of children ages 0-3. The project set out to create an impact on the way six pediatric practices in New Jersey addressed child abuse and neglect prevention by first studying each practice's structure and processes and then by tailoring interventions accordingly. The session offers ideas of new ways of including anticipatory guidance topics in well child visits.
The first phase of the project was implemented to improve adherence to new processes of care using the Multimethod Assessment Process (MAP). The MAP entailed collecting qualitative information via intense observation, formal and informal interviews with staff, and shadowing patients through well-child visits. The data was synthesized and a summary describing the practice environment, revealing areas of strength and limitations, and illustrating relationships among individuals was presented to staff. In phase two, a series of team meetings allowed staff members from all areas of the office to engage in a reflective adaptive process with the help of the research facilitator to improve work relationships, communication, and patient care processes. The research facilitator then introduced seven healthy child development modules that were incorporated in the well child visits. Each module has a variety of tools that can be used in the office.
